The real culprit turns out to be a rare and often misunderstood disease called Cutaneous Rosai-Dorfman Disease (CRDD). This condition, a variant of sinus histiocytosis, is characterized by the accumulation of histiocytes in lymph node sinuses and various tissues, including the skin. With its diverse and often misleading symptoms, CRDD can easily be mistaken for other dermatological conditions, as was the case with our patient.
CRDD, though rare, is not exclusive to any age group or gender, and its manifestations can vary greatly. From firm, reddish-yellow papules to tumor-like masses, especially on the face, it can mimic a range of skin conditions, including xanthoma, Kaposi sarcoma, and even cutaneous lymphoma. The initial histopathological examination failed to detect the key feature of CRDD - emperipolesis, where intact inflammatory cells are engulfed by histiocytes. It was only after a deeper examination of the tissue that the characteristic features of CRDD became apparent.
